AgeWell PACE’s unique and holistic model of care helps older adults with chronic care needs and their families thrive, by supporting them to remain and be served in the community. Our comprehensive and coordinated services allow us to monitor health changes, providing the needed care and preventing hospitalizations. Our interdisciplinary team works together with participants and caregivers to arrange and discuss services.
