Find out how to enroll in AgeWell PACE.
Do I Qualify?
At AgeWell PACE, we provide care and coverage to seniors who wish to remain in the community, and can do so safely, with assistance and support.
To enroll at AgeWell PACE, you must meet the following requirements:
- 55 years of age or older
- Living within the AgeWell PACE service area (see map)
- Eligible for nursing home care.
- Able to live safely in the community with AgeWell PACE services.
Enrollment Process
Step by step process
Intake and Eligibility
Once we confirm you meet the basic requirements to join AgeWell PACE (55 or older, are nursing home eligible, and live in one of our service areas), we will schedule a visit to explain the program. During this visit, you will learn if you meet the medical and financial requirements to join AgeWell PACE. (Read more about eligibility and costs.)
Most of our AgeWell PACE participants have both Medicare and Medicaid. If you have questions about your current benefits, please call (707) 615-4437, TTY:711 to speak with our intake nurse.
Assessment and Application
If it looks like you qualify for the program, our intake nurse will help gather your medical records from your current care provider. You will also have a comprehensive medical assessment by an AgeWell PACE doctor, social worker, and other members of our care team. If the state determines you don’t qualify for PACE, you’ll be notified and will have an opportunity to appeal.
Enrollment
If your PACE application is approved by the state, we welcome you to enroll in AgeWell PACE on the first day of the next month. Download the enrollment agreement
Throughout the enrollment process, AgeWell PACE works closely with you and your family to finalize all paperwork. You also get to meet your medical team to personalize your AgeWell PACE care plan. For more details, and information, call us at (707) 615-4437, TTY:711.

Enrollment Agreement
Enrollment in AgeWell PACE is voluntary. If you are not interested in enrolling in our program, please read our enrollment agreement.
