Who We Are

We are a compassionate community of health professionals that provide person-centered care empowering older adults to live in the comfort of their own homes.

What is AgeWell PACE?

Founded in 2021, AgeWell PACE is a non-profit born of the shared commitment of two Federally Qualified Health Centers, LifeLong Medical Care and Santa Rosa Community Health, to address the health and social needs of older adults with complex needs living in Marin and Sonoma Counties. AgeWell opened its first PACE center in Rohnert Park in 2023, the only program of its kind in the area supporting older adults to age comfortably and with dignity in the community.

AgeWell PACE Founders

LifeLong Medical Care has a 45-year history of prioritizing care for older adults, beginning with the Gray Panthers who opened LifeLong’s first health center in 1976 (the Over 60 Health Center in Berkeley, CA). Among FQHCs nationally, LifeLong is a leader in developing models of care for the elderly. 

As the largest FQHC in Sonoma County, Santa Rosa Community Health sees first-hand, among their patients and community, the unmet needs of low-income seniors who lack a sufficient system of care to meet their basic needs or to manage health conditions.  SRCH was thrilled to partner with LifeLong to create a comprehensive system of care for frail older adults to age safely and with dignity at home. 

What is PACE?

PACE, or Program of All-Inclusive Care for the Elderly, is a health plan and service provider that enriches the lives of seniors and caregivers and provides a comprehensive and compassionate alternative to nursing home placement. AgeWell PACE offers elders the opportunity to continue living at home, comfortably supported with wraparound care including a skilled multidisciplinary team, Adult Day Health Care, home care and home health, a full specialty provider network, prescription drug coverage and more.

The PACE model of health care started in California and has been in existence for over 40 years throughout the United States. The program focuses on prevention, wellness, and enhancing quality of life through care and support.

What is the Cost?

Cost is not a barrier. AgeWell PACE accepts Medicare, Medi-Cal and private payment. If you have Medi-Cal, you pay no premium. If you do not have Medi-Cal, contact us to determine your premium. 

AgeWell PACE participants must receive all needed health care services, including primary care and specialist physician services (other than emergency services), from AgeWell PACE or an entity authorized by AgeWell PACE. A PACE participant may be liable for the cost of medical services from an out-of-network provider or without previous authorization (except for emergency services). AgeWell PACE is a voluntary enrollment program, participants may disenroll at any time.
