We are a compassionate community of health professionals that provide person-centered care empowering older adults to live in the comfort of their own homes.

What is AgeWell PACE?

Founded in 2021, AgeWell PACE is a non-profit born of the shared commitment of two Federally Qualified Health Centers, LifeLong Medical Care and Santa Rosa Community Health, to address the health and social needs of older adults with complex needs living in Marin and Sonoma Counties.

What is PACE?

PACE, or Program of All-Inclusive Care for the Elderly, is a health plan and service provider that enriches the lives of seniors and caregivers and provides a comprehensive and compassionate alternative to nursing home placement. The program focuses on prevention, wellness, and enhancing quality of life through care and support

AgeWell PACE offers elders the opportunity to continue living at home, comfortably supported with wraparound care including a skilled multidisciplinary team, Adult Day Health Care, home care and home health, a full specialty provider network, prescription drug coverage and more..

What is the Cost?

AgeWell PACE participants must receive all needed health care services, including primary care and specialist physician services (other than emergency services), from AgeWell PACE or an entity authorized by AgeWell PACE. A PACE participant may be liable for the cost of medical services from an out-of-network provider or without previous authorization (except for emergency services). AgeWell PACE is a voluntary enrollment program, participants may disenroll at any time.
